Our Approach to Care
Hands of Hope offers two lines of service, palliative and concurrent hospice care, for children ages 0-21 experiencing a life-limiting illness. We work closely with patients’ existing care teams to determine the right care for their needs and develop personalized treatment plans.
Home-based pediatric palliative and concurrent hospice care. Services may include medical provider visits, nurse case management, social work, child life, chaplain/spiritual care, dietitian services, hospice aide support, care coordination, and grief/bereavement support. Concurrent hospice allows eligible children to continue curative treatment while receiving hospice services.
Demographic Restrictions
Children age 0-21 with a life-limiting illness. Eligibility and the appropriate level of care are determined with the child's existing healthcare team.
