The purpose of the guide is to help you and your family connect with your doctor, other medical professionals, and caregivers through a medical home. Some information in the guide might be helpful now, while other sections will be useful in the future. The guide is intended to be a user-friendly, hands-on tool to support you and your family to move forward in obtaining and providing a medical home for your child.
